Referral Packets and Information are available from HCA Behavioral Health at 714.796.0266
- The Care Coordinator submits the Referral Packet to Health Care Agency (HCA) central Service Chief Contract Monitor for review
- HCA approves referral and submits to EmploymentWORKS
- EmploymentWORKS staff reviews referral information and coordinates enrollment with HCA Coordinator, and calls the client to schedule an Orientation
- The Health Care Agency Care Coordinator and the EmploymentWORKS Staff work together to provide on-going support to the program participant
